COVID-19 Update: November 6, 2020

Greetings from the Center! The Center for Family Outreach will remain closed to the public until January 4, 2021. If it is safe for staff to return we will announce our return through social media and our website. You are important to us! Please reach out to us as we are still here for you, just in a virtual platform! Please be safe and stay connected!

We have three options for you to reach out to us:
  1. Call our phone number 970.698.7262 which allows you to access our staff directory.
  2. Fill out information our JOTFORM, which will come to us immediately through our email system. Under the programs and classes column, you will find the community referral and then a self-referral form, called JOTFORM. The JOTFORM comes to us immediately and we will respond to you in two days. If you have not heard back from us, please reconnect with us again. 
  3. Poudre School District and Thompson School District Counselors and Mental Health Crisis teams have access to our cell phone numbers and can reach us quickly if you need assistance.

March 30, 2020: 
The Center for Family Outreach is joining our community partner’s in attempting to slow the transmission of COVID-19 for our staff, students, families and community.  Our offices will be closed to the public for both Fort Collins and Loveland communities until further notice. Our staff is committed to providing services remotely from home, utilizing virtual platforms and consolidating services, for our students and their families.  Case managers and Specialists will be in contact with families, weekly, by phone or through email communication. We encourage all students, families and community partners to leave emails or phones messages through our IMPACT phone system, which allows staff to receive multiple messages. Due to this health risk we are encouraging our staff to telework and be responsible and limit any unnecessary face to face contact.

Enclosed you will find emails for our staff and their phone extensions so you all can communicate with our agency  Our goal is to make this transition as smooth as possible for our youth, families and community. We appreciate everyone’s patience, as we attempt to deliver services differently, than ever before.

As a result, we are taking the following steps immediately and our internal protocols, goes as follows.

  • Case Managers/Specialists will be in contact with students and families either through the IMPACT message system or email platforms. Staff will be checking messages daily to maintain programing with our families.
  • Program areas to be addressed will be case management, UA’s, family conflict, class assignments, therapy referrals and progress, and service learning projects.
  • All outside fundraising events will either be postponed and reviewed by the board of directors.
  • Service Learning programs, will be monitored by the Service Learning coordinator. 
  • UA monitoring is the responsivity of the case manager, if it is required by the District Attorney’s Office.
  • The assigned case manager will monitor class homework for assigned classes.
  • Case managers will be responsible for contacting other agencies that are involved in the case. This could be SRO, health agencies and governmental agencies
